Resumen: Kidney transplant is the optimal treatment for renal disease according to biomedical criteria, but the technology is highly expensive. The aim of this article was to examine the economic hardships experienced by kidney transplant patients and the impact on their lives, specifically when they lack social protection in health. The article reports on a qualitative study conducted in Mexico. Twenty-one kidney transplant patients participated. Semi-structured interviews were performed and submitted to content analysis. Patients experience extreme economic hardship due to the high cost of renal therapies, particularly medicines. Such economic problems adversely affect their condition, since many patients report difficulties in maintaining their immunosuppressant medication, attending medical appointments, and curtailing household expenditures, further aggravated by loss of earnings. In conclusion, kidney transplantation is associated with patientsí impoverishment when they lack social protection in health. A protection system is urgently needed for this group. Guerrero, R.",,"2009",,"Introduction: due to the unsatisfactory results obtained with posterior instrumentations in cases of vertebral tumors and to the economical limitations of our patients for acquiring the systems available in the market, a vertebral body prosthesis to stabilize the spine after corpectomy was designed. Objective: to describe the structural and functional characteristics of the JR prosthesis, the biomechanical assays in cadaver and the surgical technique on the patient. Methods: firstly, it was performed a detailed anatomical study of the vertebral bodies. A design of a modular, anatomical and expandable device was obtained. Its components once assembled make an implant with expandable cage and plate dual function. Afterwards, a biomechanical assay was performed in a cadaver and the device was implanted in a patient with vertebral body metastasis. Results: the radiographs extracted from the cadaver after implant location did not show loss of fixation. When the cadaver was raised, flexion moments were generated laterally with levers 80 cm long; therefore, the prosthesis was demanded in its most vulnerable point with a force of approximately 588 N. With rotation, flexion and extensions forced, stability was preserved and no movement was observed. The device was implanted in a 50-year-old woman with metastatic thyroid cancer affecting L3. Pain improved in the postoperative immediate, as well as its motor function, which allowed her to walk with a stable and painless spine for seven years. No implant failure was observed. Conclusion: the dual cage function and plate integrated in a single device offers a mechanical advantage, compared with the cages and plates used separately, since the prosthesis design permits to apply a distraction axial force with lateral fixation at the same time, through one implant. With regard to metabolic deficiency control, the explanation is centered fundamentally on the patient's not getting a complete diet. This could be due to a lack of information about their illness, thus becoming a determining factor in their treatment. The purpose of this study was to analyze the level of knowledge, attitudes, and dietary education practices by means of a survey using the likert scale. This was applied by means of a direct and codified interview directed to patients in a Primary Care Health Unit from the Mexican Social Security Institute (IMSS) in Guadalajara, Jalisco, Mexico. 201 Type II diabetic patients, selected from a systematic sample, were studied. The average age was 54 and predominantly female (a ratio of 1.7 to 1). The sample had an academic background not surpassing the elementary school level, and their occupation was that of housewife. Their central glycemia level was, on the average, 191 mg/dl with a standard deviation of 120. The average number of years of having suffered from the illness was 5.5 with a standard deviation of 5. Ninety per cent of the diabetic patients take oral hypoglucemiants. Although there does exist a high level of knowledge and attitudes about dietary education, there exists a low level of educational practices. Gonzalez-Alvarez, V.",,"2005",,"In this work, an optimization method is implemented in an anaerobic digestion model to estimate its kinetic parameters and yield coefficients. This method combines the use of advanced state estimation schemes and powerful nonlinear programming techniques to yield fast and accurate estimates of the aforementioned parameters. In this method, we first implement an asymptotic observer to provide estimates of the non-measured variables (such as biomass concentration) and good guesses for the initial conditions of the parameter estimation algorithm. These results are then used by the successive quadratic programming (SQP) technique to calculate the kinetic parameters and yield coefficients of the anaerobic digestion process. The model, provided with the estimated parameters, is tested with experimental data from a pilot-scale fixed bed reactor treating raw industrial wine distillery wastewater. It is shown that SQP reaches a fast and accurate estimation of the kinetic parameters despite highly noise corrupted experimental data and time varying inputs variables. A statistical analysis is also performed to validate the combined estimation method. Finally, a comparison between the proposed method and the traditional Marquardt technique shows that both yield similar results; however the calculation time of the traditional technique is considerable higher than that of the proposed method.
